A PHONE OR FACE-TO FACE CONSULTATION IS REQUIRED (FREE OF CHARGE) PRIOR TO THE MEDICAL ASSESSMENT. THIS IS TO ENSURE THE DOCTOR CAN EXPLAIN EVERYTHING REQUIRED, ORDER ANY PATHOLOGY NEEDED, ANY REFERRALS AND DISCUSS PROCEDURE WITH PATIENT.


**PLEASE ASK YOUR EMPLOYER FOR ’THE RAIL WORKER ASSESSMENT FORM’ AND ASK WHICH CATEGORY IS REQUIRED (CATEGORY 1, CATEGORY 2 OR CATEGORY 3). IF NEEDED, DR HAQUE WILL ALSO SEND FORMS VIA EMAIL THAT WILL NEED TO BE FILLED PRIOR TO THE DATE OF THE MEDICAL ASSESSMENT.
